6 Treatment Planning

In treatment plans that must consider respiration-induced organ motion, it is important to think carefully about the relationship between the amount of respiration-induced organ movement and the irradiation time of the synchronous irradiation method. For margin setting, it is necessary to add the inter-fraction and intra-fraction as the ITV margin in addition to the normal margin. Currently, based on respiration-synchronized 3DCT data, an irradiation plan with an ITV margin added to the target is set up. The ITV margin may be set for each part on the basis of previous reports, on the experience of each facility, or by setting the ITV margin for each patient on the basis of 4DCT data; the latter makes it possible to measure the shape of the tumor in the full respiratory phase. In addition, the DIR technology (Fig. 2) can support the treatment planning by evaluation of the 4D movement of all point in patient’s body.
